Call OnePath at 1-866-888-0660. We’ll help you fill out the enrollment form over the phone.
Review and sign the enrollment form. Then mail or fax it back to OnePath as noted on the form.
Before completing your enrollment, OnePath will contact your healthcare provider to confirm your diagnosis and prescription. OnePath will let you and your healthcare provider know once you are enrolled.* †

No claim for reimbursement of the out-of-pocket expense amount covered by the Program shall be submitted to any third party payer, whether public or private.
